For a child who is still under the age of
16, you can submit an online or postal application and please select one of
these two choices. Before applying, please read and complete the application.
If you haven't changed
your name after utilizing the Deed Poll to do so, enter your current name in
the new name fields on the application form, and if you have, enter your prior
name in the current name boxes. The idea is to replicate the name change from
your lost Deed Poll document.
We can modify your
replacement Adult Deed Poll to add a statement stating that you have been using
your present name for all purposes since a certain date if you are asking for an
Adult Deed Poll. If you would like this change to be made, which is something
we strongly advocate, kindly let us know via the online application form. We
need to know how and when at the very least the year you initially changed your
name. This update makes it clear to everyone you give your replacement Deed
Poll to that it is not for a fresh name change. Using old Deed Polls to track
name changes is becoming increasingly common as more people conduct family tree
research.
You have lost if your name
